Home From Hospital provides support when many people can become overwhelmed: following their discharge from the hospital or when there’s a risk of being admitted. We can offer help in a range of ways including the collection of shopping or prescriptions, helping to ensure the home is safe and comfortable, or managing appointments and supporting people to pay bills.

We know this time can be a particularly stressful one, especially for those who don’t have the support from family and friends. The Home From Hospital service provides short term help whilst the individual recovers and while other more long-term support is put in place if required.

Home From Hospital makes a big difference within our community, aiding individuals in need both physically and emotionally whilst relieving the stress placed on other caring services working across the district.
